If you look closer at the screenshot, you can see you have uploaded the exhibitor-information_export folder and then inside of that you have another folder exhibitor-information. The _exported folder is never uploaded. You upload the contents of that folder.

So exhibitor-information needs to be in the root folder of your site.

I added within the VSD.
I've added on Filezilla.
I just don't know what I'm missing. Thinking I'll stop thinking for today & start fresh tomorrow.

Hi Barbara, Don't add the files to vsd. FTP them up to your site with your ftp app.
You need to upload the contents of the myformname_exported to your site so you maintain the file structure that the formbuilder created. This means in your ftp app you will open the myformname_exported and upload the myformname.php and the entire myformname folder that's inside the myformname_exported.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.
