For page design using VSD, I had used mymenu earlier, but I did not like it. So I removed it and used another menu object. However, when I upload the files again with my new menu object, I was prompted that mymenu could not be found. I also had similar experience with other graphic files also.

How can I completely remove the ghosts of deleted objects and never hear from them again in whatever circumstances, including uploading.

Hey Maizy, if you haven't already done this, there's a Remove Files option in VSD to clean out what you no longer want uploaded. Deleting the HTML Tool box w/ the code doesn't remove the related files from the project. The "Remove Files" option has changed with the more recent VSD 5.8 version. For previous versions, you go to:
Settings -> Additional Files -> Remove Files
5.8+:
Settings -> Remove Files
Check off the files to remove from the project and click OK.
As always, I'd recommend upgrading to the latest & greatest, currently 5.81.
