My host is JustHost. I'll contact them and ask about filling in the fields in the link Scott posted. I tried a different email addresss in my form, and got the same result.

I like using S-Drive. I have a form uploaded there already and it works great. I really don't need to upgrade my S-Drive account at this time to add another form. But you're right, Scott, S-Drive does work well.

Amy I host with JH and can confirm the forms worked first time every time for me, so unlikely its a hosting issue

Only problem i had (Scott might remember this) was i selected a wrong option for emails something like, replacing it with [ client ] instead of the actual email address cured it.

I contacted tech support and they helped. I ended up replacing the "from" on the Notification Message with [email10] and it went through. Don't know why that worked, but it did.
