I would like to create a web form, which puts the result into feilds of a document and:
-Emails to me, and
-Displays the pre-made document online to the user (thank you page)

Example:
Form:
What fruit do you like: *text box entry* (user selects apple)
What's your favorite color: *drop down options* (user selects red)

Document output: We'll send you a delicious red apple, thanks! (this is displayed on screen and emailed to me).

How can I do that?

The built in page is fully customizable.:)
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

That is a hard-coded block. If you want to customize that part, remove [_form_results_] and then insert each element from the dropdown list at the bottom of the dialog.

Look down a little and click the "Configure Email Message" You'll then be able to delete the [_form_results_] and pick individual elements to display on your page.:)
