How to get the return I want on Web...

I have a web form that includes primarily checkboxes and radio buttons. Because the results are getting dumped into a database, I'd like the checkboxes to return a "0" even when they are not checked. I have the results being returned to me via e-mail in a database format, so I'd like to get a response for EVERY checkbox whether checked or unchecked. Is this possible? I would also like to ensure that I get a placeholder for radio button questions in the event that they are not responded to. If this is possible, how can I modify my form? (currently I receive no type of placeholder for unchecked responses or omitted radio buttons)

You can set that up in your viewing script to look up values that are NULL in that database and show them as 0
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.
