Still getting all the error messages.
What goes on the "Storing Results" page?
I run Linux on my server and not MySQL.
Do I make a folder in my httpdocs files on my server to publish to?
And exactly what do I copy and past a form on a webpage at.
I haven't seen anything that would let me copy it yet.

That is the easiest part.
To publish your form to your own server, go to the File Menu > Export > Manual Server Setup. This will then generate all of your form files that you need to upload to your server. Simply take those files and upload them with any FTP client.

I use IP switch FTP client.
I can load them to the root directory along with the httpdocs folder, I can load it to a folder in the httpdocs files.
Does it need it's own folder and where does that folder need to be?

You can load it anywhere you like as the location is completely up to you.

I made a folder called "ContactForm" in my httpdocs folder on my server.
I loaded the form in the folder, and copied & pasted the code in the page code I wanted the form on.
Here is what i'm getting.....

There is no need to make any folder though. Simply upload the contents of the _exported folder and that is all you need to do.

It still wasn't working for me.
SO, I deleted the Contact Form folder in the httpdocs files, and just loaded the ContactForm_Exported folder in my httpdocs files.
Reloaded the code on my webpage and still nothing.
Here is the code I'm putting on my webpage...

As Scott keep on saying, You need load the files that are in your ContactForm_Exported folder. There should be folder call ContactForm and a ContactForm.php. That's the two things that needs to on the server where your web page is located. The code that you have posted on the post before is correct. You don't need to make any special folder just load the files are in the ContactForm_Exported folder.
It will work when the proper files are on your server.
