My question is how can we do add css buttons created with the standalone application "Coffeecup Button Builder" to my visual site designer?

You create and HTML box to put the code in that Button Builder gives you. Then you can do 1 of 2 things. You can FTP the files to your server, or you can use Edit/Add files to add those files to VSD and they will upload when you publish your site. The advantage to using Edit/Add files to put the files into VSD is that you will be able to preview your site with the buttons before you publish it.
