Looking at your forms, I see all of them were created with the same version/build of Form Builder. One important thing to remember that with Form Builder, the scripts do not automatically update themselves. So if a form was working and then stopped, something on the server has changed. Any number of things from security updates to new installation versions of PHP upgrades could factor in this.

One thing I can see on your form and which you mentioned about the Reply-To is that you are not using an Email Address Element on your form. Without collecting an email address and hard-coding it to always be your prophat@, some servers may potentially treat the email as SPAM. By adding an email address, not only will it fix the Reply-To it may also help with the missing emails.

I would start first by adding an email address to your form and testing it out. If it works, GREAT! If you still have problems, publish that same form to our servers (S-Drive) and test it out there. If the emails arrive when using our servers, that shows there is some issue where your form is currently hosted at.

my server administrator states all emails are being sent. we have begun to receive them all except for those sent to my msn.com account, we think msn is filtering or blocking those emails

i'm sure msn.com is filtering these emails because i have not set my reply-to address. they were filtering all those emails into my junkmail folder at one time, now it appears they are blocking them entirely. i have searched the forums, and the knowledge database, as well as searched google for answers on how to rectify that. but i am coming up blank on all fronts

You still must add an email address to your form. Once you add this to your form, you will then see it populated under the Reply-To. I would also set that as Required and then test your emails again and see if that helps with MSN.

Have you also tried publishing your form to your S-Drive account to see if the emails arrive when hosted on our servers?

I'm having the same issue since upgrading to version 2.5 build 5437. I opened up a form I use once a year and just modified the dates of events and such. Now no notifications. I did the php version check and noticed the host is 5.2.17 and the requirements for Web Form Builder is 5.3. Could this be the cause? Should I look for anything else?

Previous version of the builder was 2.4 build 5318. I did not change any of the notification addresses and the from is the same domain as the site.
BTW, the PayPal portion works great, so this is a minor inconvenience. Actually, even the save to csv works, so the only difference is the email notification the form was submitted.

Thanks!
Bill

Yes, version 2.5 does require PHP 5.3. You will need to upgrade your PHP installation. Most hosting control panels have an option through your dashboard for that. If not, contact your hosting provider tech support and they would be able to do this for you.

OK, I've confirmed that provider now says php is now at 5.3.24. I did a test and no notification was sent. Is it recommended to re-save the form and post again? Is there a format to use if multiple recipients are going to receive the form? E.g. me@domain.com, you@domain.com or me@domain.com; you@domain.com. Comma v. Semicolon.

And what if I have the option to go to php 5.4? Recommended?

Thanks!
