Forms are a good item to use on your site. However, you should try to keep them short and ask only relevant information. Users are faced with forms constantly, and filling out these long forms can sometimes turn users away from your site because they don't have the time or patience to fill out your form.
Try to make it easier for users to fill out the form by cutting out any questions that really are not necessary. You can also make sure that your form supports auto fill so that users can automatically fill in the information such as Name, Address, etc. Lastly, don't make fields required unless they really are!
