Trying to get form results to save to...

Hi. I have created a form and tested it (without setting it to save to a database) and it works. Now I want my form results to save to an SQL database. So I completed that section under the STORING RESULTS in settings. But now I get this error after completing the form and clicking SUBMIT:

Warning: [ err 2/line 74/datasavemysql.cls.php] PDO::__construct(): php_network_getaddresses: getaddrinfo failed: Name or service not known
Failed to open database [2002] SQLSTATE[HY000] [2002] php_network_getaddresses: getaddrinfo failed: Name or service not known.

Any ideas what I did wrong. I created a database on the server but did not create a table. I did include a table name in the Table name field in STORING RESULTS.

Any help appreciated.

It looks to me like the form is not connecting with the database. You may want to double check your database settings in WFB.
