Form is on opportunities page. I looked at it in Internet Explorer and it doesn't show up? Safari and Google Chrome look fine. This won't work if it doesn't show up in some browsers. Any ideas??

Shows just fine for me in IE.

Try clearing your cache in IE and refreshing the page and see if that is the culprit.
