Adding new form to website

I just created a new form and following the instructions, it says to copy the code below and paste it where you would like it to appear in your website. I literally copied it and pasted it on my contact page. Obviously I have done something wrong as this is what it looks like : <script type="text/javascript">document.write(unescape("%3Cscript src='http" + (document.location.protocol == 'https:' ? 's' : '') + "://www.coffeecup.com/api/sdrive/forms/form.js?name=contact%2520form%26slug=37249%26height=623%26crossdomains=true%26rand=" + Math.floor( Math.random() * 1000000000 ) + "' type='text/javascript'%3E%3C/script%3E"));</script>

What do I do next?

Hi
Not sure I understand what page youre looking for. I tried to add the form to this contact page, but the code came up, but no form. Does this make sense?

What is the URL to the page? We need to see it.

Hi, I followed the directions exactly and this is what appears after I add the HTML button. What am I doing wrong?

You can not view the form in preview. You have to upload the website to see it.

Your contact page was last uploaded Sat, 15 Nov 2014 17:40:58 GMT. :P

Once you do that you should be good to go.
